The purpose of the conference and summit is to be the premier educational event for hospital practitioners interested in laying the foundation to expand pharmacists’ roles in patient care in ambulatory settings.
During the conference, attendees will identify and expand opportunities for pharmacists to interact directly with patients in the ambulatory setting and to assume responsibility and accountability for patients’ and health system outcomes. At the conference, practitioners will utilize research that aims to further validate the clinical and economic outcomes of pharmacists’ contributions to patient care in ambulatory settings; and provide a system for prioritizing patients in need of higher level drug therapy management.
Our goal is to create a vision for forward thinking pharmacy practice models that will ensure pharmacists are members of the ambulatory healthcare team who are responsible and accountable for patient and healthcare system outcomes.
