I can’t believe that 6 years is finally over, and I am now officially Dr. Desko!  I graduated from Ohio Northern University just over a week ago, and now it seems that life has really kicked into high gear!  My very last rotation in pharmacy school was at the FDA, which was a really great experience.  I learned so much about what the FDA does and feel that I am so much more informed about the regulatory aspect of pharmacy than I was before my rotation.  If regulatory is something that appeals to you, I would highly recommend applying for the FDA rotation.  I also want to discuss a few things that you may not think about until graduation is almost upon you, and hopefully give some of you a head start for future planning! I agree completely with Kelly's insightful blog. It seems that we are constantly struggling with justifying our services. When we think we have finally got it, something new comes along and we have to adapt. There is always going to be something new. Pharmacists must be ready for it and ready to lead. An example of this in our clinic has been transitions of care and decreasing hospital readmission rates. We included the PGY2 Ambulatory Care resident as a quality improvement project. The team actually wanted a PA/NP initially but how can you say no to a "free" resource. It also gave us a chance to show that a pharmacist is really what was needed to provide key medication reconciliation interventions. We collected information on multiple areas but examples include, access to care and how we were able to decrease time to follow-up compared to usual care. We also collected examples of interventions and shared those twice a month at our team meetings. When the new TCM CPT codes became available, we were ready. We only had to make minor tweaks to our current system to make them work. Although pharmacists can't bill directly for those codes b/c they must contain face-to-face time with a physician, we have built the business model to allow a portion of the visit reimbursement to pay for the pharmacist's time. At our RDC discussion of the preliminary Task Force on Organization Structure recommendations this past weekend, I had an idea of merging two aspects-engaging past leaders and state affiliates. What if ASHP had past leaders become liaisons to the state affiliates after their term of service is completed? Leaders rotating off of the board, Section Executive Committees, Council leadership, etc could volunteer to become liaisons to their respective state affiliate. Responsibilities could include mentoring their boards or members on leadership, ASHP involvement, and advocacy. They could be speakers or provide discussions at the state meetings to the pharmacists, residents, students or technicians on leadership, mentoring, or ASHP activities. There could be a term limit for this appointment and multiple past leaders could serve at once to create a team potentially. This could result in a stronger cycle of communication and member involvement between the national and state organizations as well. [more]

I am at the end of my third year of pharmacy school which means that over the past week I have had a lot of “lasts”: my last final exam, my last didactic lecture, my last time sitting in my lecture hall seat. Many of my classmates were, appropriately, excited about these lasts, but I have found that what I am truly excited about are the “firsts” towards which we are all quickly moving. [more]
