Sharing Details of Pharmacists getting involved in new CMS rule, HCAHPS, Core Measures

Hello to all Pharmacists and Technicians-

As part of the sharing and networking for the Section of Inpatient Care Practitioners I encourage you to post and share what you are doing to get involved in:

1.  The new CMS rule supporting pharmacists as part of the medical staff (how are you navigating what your state will allow us to do).
2. HCAHPS
3. Core Measures
4. Other areas where pharmacists and technicians can take accountability for our hard work and care of our patients.

One of our new PGY1 residents is developing her project to assist nurses on our cardiology units to improve HCAHPS scores related to medication information and side effects. We have printed materila on several cardiac drug classes for patients and the resident will train P4 Pharm.D. students to do verbal counseling to reinforce the information given at time of first dose by the nurses. The HCAHPS scores will be monitored throughout several months to determine the level of improvement after the intervention

1. CMS rule- I have emailed our ASHP state affiliate (NCAP) to inquire about how we can work as a state to review what the state will allow related to this new rule and if we as a state society need to advocate for modifications at the state level to allow for a more progressive pharmacist involvement
2. HCAHPS- we have a heart failure pilot where our pharmacists are preparing the discharge instructions/medication list for the heart failure patients and counseling all these patients. I presented this to our COO and requested that pharmacists be responsible and there is a metric on the corporate balanced scorecard for this.
