Are you a clinical practitioner seeking to enter into the ambulatory care space or potentially expand your current ambulatory care pharmacy services? Is your institution seeking to implement a transitions of care program to help patients successfully migrate between the inpatient and outpatient settings? Consider applying to the Spring 2018 A3 Collaborative cohort!
The Alliance for Integrated Medication Management (AIMM), Apexus, and ASHP joined forces to offer a unique learning experience designed to help healthcare providers and organizations succeed in the new era of value-based payment programs. After a successful launch in 2017, the A3 Collaborative is offering a third learning cohort (to begin in May 2018).
Participating organizations will be provided with access to resources and tools to build the business case for an effective Comprehensive Medication Management (CMM) strategy that focuses on improved health outcomes, better patient care, and cost-effectiveness. The A3 Collaborative supports organizations in making CMM a critical part of their strategic plan as well as their responses to the larger, quality strategy reform for value-based care programs evolving within healthcare models.
Applications for the Fall Cohort are now available online (deadline is April 30th). Participation is offered at a cost of $9000 per year plus the cost of team member travel to in-person events (held prior to ASHP Summer and Midyear Clinical Meetings). Scholarship support is available to help offset the cost to $3000. Apexus is offering a limited number of scholarships to organizations that demonstrate a commitment to establishing or expanding comprehensive medication management services.
