3 Comments - There is no new magic formula for providing patient care within the model of an ACO or PCMH. Pharmacists have been providing patient care in this capacity for years within other organizations, they just have not called themselves ACOs or PCMHs

The advancement of the profession has created new and exciting opportunities enabling pharmacists to provide optimal patient care. As mentioned in the Oath of a Pharmacist , “Pharmacists will embrace and advocate changes that improve patient care.”
Working with trained clinicians, I felt that I could better manage, make decisions and communicate patient care therapy utilizing current guidelines.
